Evans and Associates team is happy to announce that beginning September 1 st , 2015 North American Benefits Company (NABCO) can a ccept payments for your Occupational Accident Insurance premium by using a major credit card (MasterCard, Visa, Ameri can Express, Discover). Paying by credit will make the payment process simple, fast and above all secure. If you choose to pay by credit card, your payment can be processed by calling our GAPP/TAPP payment processor at 800-537-4565 or 610-995-0169. When calling, please have available the information listed below or you may fax the premium invoice to 610-995-0181 or email to gappptd@nabenefits.com prior to your call. ï‚· Policy Number ï‚· Policyholder Name ï‚· Premium Due Date ï‚· Monthly billing detail, such as total number of employees, monthly covered payroll per class (if applicable), total premium per cl ass and total payment amount due. In addition to the above information th e payment processor will need to know: ï‚· Type of Credit Card (MasterCard, Visa, American Express, Discover) ï‚· Name as it appears on Credit Card ï‚· Current Address ï‚· Credit card number ï‚· 3 Digit Security Code on back of card ï‚· Card Expiration Date ï‚· Email address (optional) A receipt of your transaction can be emailed to you on ce the above information is entered into our secure network and the transaction is complete provided we obtain your email address.
